At Meadows Family Doctors, we are committed to providing the highest quality of care to you and your family. We value your feedback—whether it is a compliment, a suggestion, or a complaint—as it helps us continually improve our services.

How to provide feedback or make a complaint:

• By Phone: Call us on 08 7190 7972 and ask to speak with our Practice Manager or your doctor.

• At Reception: Complete a Feedback Form, which is available at our front desk. You can hand it back to our reception team or place it in our secure suggestion box.

What happens next?

We take all complaints seriously. Your concerns will be treated confidentially, and we will work with you to resolve the matter promptly and fairly within the practice.

External Complaints

We prefer to be given the opportunity to resolve complaints directly with you. However, if you feel your concern has not been resolved to your satisfaction, you have the right to contact the independent state health complaints agency:
